Reach Services - Helping Adults with Disabilities

In partnership with ELARC, SGPRC, RCOC, Harbor Regional Center, and Lanterman Regional Center — and providing support to individuals in the Self-Determination Program.

REACH ADULT COMMUNITY DAY PROGRAMS

This program gives individuals the opportunity to be active members of their community in a group setting (1 staff with 3 participants). Participants engage in a variety of activities based on their personal goals, such as self-help skills, developing social and recreational abilities, and continuing education. Examples include visiting libraries, volunteering, attending fitness classes, and practicing life skills in real-world settings.

In addition to Community Day Services, Supplemental Services helps implement specific objectives outlined in an individual’s IPP while allowing them to continue participating in their day program. Support may include assistance & training in daily living skills, developing socially appropriate behaviors to replace challenging ones, & accessing community resources

This program provides a personalized alternative to traditional day services, offering 1:1 support in person, remotely, or through a combination of both. It focuses on individualized goals in areas such as employment, volunteering, post-secondary education, and community integration. Services are limited to 92 hours.

REACH provides transportation for participants receiving 1:1 support within a 10-mile radius and for those in a 1:3 ratio within 8 miles. Transportation is available for individuals using wheelchairs who can transfer into a vehicle and use a foldable wheelchair.

REACH RESIDENTIAL PROGRAMS

This service supports individuals in living in their own homes, with assistance available as often and for as long as needed. It may also help with making and managing life decisions, building strong and lasting relationships, choosing where and with whom to live, and having control over the look and feel of their home environment.

Provides support in essential daily living skills such as cooking, cleaning, shopping, meal preparation, money management, using public transportation, personal hygiene, and accessing medical or community resources. In addition, it may offer functional skills training and ongoing support to help individuals transition to or maintain or increase independent living.

COMING SOON!

New service designed for individuals exiting work activity programs, subminimum wage settings, or within two years of exiting secondary education to achieve or advance in competitive Integrated employment (CIE). Two new services are available through Pathways, a Career Pathway Navigator (Navigator) and a Customized Employment Specialist.
